You must first inform the opposing team that you are forfeiting this game. Forfeits submitted 72 hours prior to the scheduled game will not be assessed referee fees but forfeits submitted within 72 hours of the scheduled match time will be assessed referee fees.

Only coaches, team managers of either team are authorized to request a game forfeit. If you are a coach/assistant coach and your name is not on the list, please ask your club registrar to add you as the coach of your team. Your full name, email address, telephone, home address, your team and your role (Coach, Assistant Coach, Team manager) are required.
